How we read the Minnesota Department of Health record
Every facility on this site links to its licence record, and 4,388 of the 5,098 carry a score. The score is built from one source only: what the Minnesota Department of Health posted about that licence. Nothing an operator says, and nothing about being featured, moves it. If you want a person to read a record with you, call (888) 282-PALM.

Since August 2021 every assisted living facility in Minnesota is licensed under chapter 144G and appears in the MDH provider directory with two dated lists: posted licensing surveys, and Office of Health Facility Complaints (OHFC) investigations with the regulator's own conclusion. Palmelle reads that directory, joins it to the licence roster, and refreshes the result; the reconciliation date is in the footer of every page.
What counts
- A substantiated maltreatment finding — an OHFC Vulnerable Adults Act investigation MDH concluded was substantiated (neglect, abuse or financial exploitation of a resident). Weight 6.
- A substantiated licensing complaint — an OHFC compliance complaint MDH substantiated. Weight 2.
- A posted licensing survey — MDH posts these when it issues correction orders; a random sample of 35 posted surveys in September 2026 were all correction-order letters citing 144G violations. Counted once, however many orders it carries. Weight 1.
Complaints MDH found unsubstantiated or inconclusive weigh nothing. A finding five years old counts half. The total is divided by the facility's licensed beds and by a fixed five-year window, so a 5-bed home and a 150-bed community are held to the same rate, and that rate lands in a band from 3.0 to 9.5.
What "Not yet scored" means
MDH has posted nothing for that licence: no survey, no complaint finding. 710 of the 5,098 facilities in this ring read that way today, most of them licensed within the last year. It is shown as words, never as a zero, because absence of a record is not a finding.
What the score does not know
Staffing, turnover, food, how the building feels at 7 pm, whether the administrator answers the phone. Those are what the visit and the calls are for, and what Help Me Choose collects. The score tells you what the state found; it does not tell you where to live.
Pets, and other things the state does not publish
Pet policy, rates, waiting lists and respite availability are not in any MDH record. Palmelle asks each operator directly and records the answer with who said it and when; a facility that has not answered shows no pet line at all. That is why some listings here say "Pets: yes" and most say nothing.
